New Event & Sample Request Form

Please be advised that all orders and event requests require approval before making any commitments. While submission of this form is necessary, it does not automatically signify approval for the respective event. We will reach out to inform you of the status of your event once it has been reviewed and approved.

All orders are fulfilled in the order that they were received and are subject to availability. Please try and submit more than a week before the start of your event, to give us time to pack and ship. If you have a last-minute lab, we will do our best to fulfill last-minute requests. All sample requests are subject to the availability of product.

Return samples within 48 hours of the end of the event using the return labels provided.

For SPM sawbone demonstration-only events, we will not provide fully equipped sports trays.

Distributors will be provided a relevant set of sports medicine sample product at no cost during onboarding upon request.
Beyond initial samples, Distributor Sales Representatives may be requested to share in the cost of samples which will be determined as the situation presents itself.
